چکیده

Numerous studies have shown that biventricular pacing (BVP) improves symptoms and exercise capacity in patients with congestive cardiac failure (CCF) and left bundle branch block (LBBB). The mechanism of symptomatic improvement has been attributed to changes in ventricular synchrony. However, the degree of asynchrony is not a good predictor of response. 2 The aim of this study was to assess whether the presence of a restrictive mitral filling pattern, previously shown to be a marker for diastolic ventricular interaction (DVI) in patients with CCF, might identify a cohort of patients more likely to respond to BVP.

Prognostic stratification in heart failure: what's the point?

max) in 100 men with heart failure and impaired systolic function. Patients were divided into groups based on transmitral flow patterns and E wave deceleration time, with a third group consisting of patients with fused E and A waves due to tachycardia (fusion group). Whilst E wave deceleration time (Dt) correlated strongly with maximal exercise capacity (r=0·65), per cent predicted VO2 max rema...

Left ventricular diastolic function. Doppler echocardiographic changes soon after cardiac transplantation.

In acute cardiac rejection, left ventricular diastolic function is altered, and a restrictive ventricular filling pattern occurs. Doppler echocardiographic indexes of mitral inflow have been proposed as sensitive markers of the rejection process. As rejection progresses, the restrictive ventricular filling pattern is reflected by a shortening of isovolumic relaxation time and mitral valve press...

Tissue Doppler echocardiographic evidence of reverse remodeling and improved synchronicity by simultaneously delaying regional contraction after biventricular pacing therapy in heart failure.

BACKGROUND Biventricular pacing has been proposed to improve symptoms and exercise capacity in patients with advanced heart failure and wide electrocardiographic wave complexes. This study investigated the effect of biventricular pacing on reverse remodeling and the underlying mechanisms. METHODS AND RESULTS Twenty-five patients with NYHA class III to IV heart failure and electrocardiographic...

Restrictive filling patterns in patients with reduced systolic left ventricular function: identification by velocity encoded magnetic resonance imaging

Methods The study included 41 patients with reduced systolic LV function (ejection fraction 29±12 %). All patients underwent VENC-MRI and Doppler-echocardiography to assess the transmitral and pulmonary-venous flow characteristics. Figure 1 illustrates measurements of maximal earlyand late-diastolic transmitral velocities (Eand A-waves) as well as maximal systolic and diastolic pulmonary venous...

Left ventricular diastolic function of remodeled myocardium in dogs with pacing-induced heart failure.

In patients with heart failure, decreased contractility resulting in high end-diastolic pressures and a restrictive pattern of left ventricular filling produces a decrease in early diastolic filling, suggesting a stiff ventricle. This study investigated the elastic properties of the myocardium and left ventricular chamber and the ability of the heart to utilize elastic recoil to facilitate fill...
